Attendees have the option to register online or by completing this form and returning it to LeadingAge Connecticut. The attendee registration form can be submitted to Ñurka Carrero at [email protected], Fax 203-678-4650 or mail to 110 Barnes Rd, Wallingford, CT 06492.

Paper Registration: Complete the form and submit to LeadingAge Connecticut.

Non-member Providers $160

Please note that registration is a commitment to pay in full. However, if your organization is undergoing a health inspection survey on the day of EXPO, LeadingAge Connecticut will provide a refund.
