This 6-week series equips LTC/AL teams with practical strategies to reduce fall risk, incidents, and injuries. Topics include QAPI for safety, improved risk assessments, root cause analysis, targeted interventions, care planning, and strengthening staff engagement in fall prevention.

July 22 - QAPI Tactics for Sustaining Positive Outcomes
Falls remain one of the most frequent adverse events in skilled nursing facilities and are a key focus of QAPI and regulatory oversight. This presentation examines how an effective QAPI program can be used to reduce fall risk, prevent repeat falls, and improve resident outcomes through data-driven decision making. Participants will explore regulatory expectations, performance monitoring, root cause analysis, and sustainable improvement strategies that integrate interdisciplinary teamwork, staff accountability, and continuous monitoring. Emphasis is placed on moving beyond compliance to measurable, ongoing quality improvement.

Projected Learning Outcomes/Course Objectives:
1. Describe regulatory and QAPI expectations related to fall prevention, investigation, and performance improvement in skilled nursing facilities.
2. Identify key data elements and indicators used to monitor fall trends, repeat falls, and high-risk residents within a QAPI framework.
3. Understand how to apply root cause analysis techniques to falls and fall-related injuries to identify system gaps.
4. Learn methods of developing targeted performance improvement interventions that address clinical, environmental, staffing, and process related fall risks.
5. Evaluate the effectiveness of fall prevention strategies using ongoing monitoring, audits, and outcome measures to sustain improvement.
