This 6-week series equips LTC/AL teams with practical strategies to reduce fall risk, incidents, and injuries. Topics include QAPI for safety, improved risk assessments, root cause analysis, targeted interventions, care planning, and strengthening staff engagement in fall prevention.

June 24 - Getting to the “Root” of the Fall with Root Cause Analysis
Effective fall management interventions cannot be implemented if the root cause of the fall has not been identified. This session will focus on completion of a detailed root cause analysis that will guide the facility in determining the underlying cause of falls. Sample methods for root cause analysis will be discussed along with the critical components that must reviewed for a thorough evaluation of resident fall contributing factors. Strategies for staff involvement will be explored that will lead to successful team fall reduction outcomes.
